What we're looking for is real life problems.  Problems that you might face day to day delivering healthcare in the community. On the ward, in the back of an ambulance, wherever people have health care needs, it might be a problem that you as a patient have in your own home. What we're not looking for is a long, detailed description or the start of a journal article that you hope BMJ will publish.
